Birth & Death Registration
This section records all births and deaths within municipal limits and issues official certificates used for
school admission, legal identity, pension, insurance and other legal purposes.

Why Registration is Important
- Birth certificate is the first legal identity document of a child.
- Death certificate is needed for pension, insurance, property and bank matters.
- Registration within time helps avoid extra documents, affidavits and late fees later.
After a Birth – What Family Has to Do
If baby is born in a hospital / nursing home
- Collect birth intimation form / slip from hospital.
- Check parents’ names and address are written correctly.
- Visit municipal Birth & Death counter or authorised centre within the time limit.
- Give baby’s name (if decided), parents’ details and sign the form.
- Apply for Birth Certificate copies after registration is completed.
If baby is born at home
- Get a note from a registered doctor / health worker wherever possible.
- Bring parents’ ID proof and address proof.
- Visit Birth & Death counter in the municipality covering your area.
- Fill the birth report with date, time, place of birth and parents’ details.
- Apply for certificates once entry is made in the register.
After a Death – What Family Has to Do
If death occurs in a hospital
- Collect death intimation and medical certificate from hospital.
- Keep ID details of the deceased and informant (family member).
- Visit municipal Birth & Death counter within the time limit.
- Fill death report form with date, time and place of death.
- Apply for required number of Death Certificates for legal work.
If death occurs at home / outside hospital
- In natural death, obtain certificate from registered doctor who last attended the person.
- In accident / police case, follow instructions of police and hospital first.
- Carry these documents with ID proof to Birth & Death counter.
- Report the event and submit death report form.
- Apply for certificates after registration is done.

Original documents may be checked at the counter. Self-attested photocopies may be required for office records.
Time Limits, Late Registration & Corrections
- Normal time limit: Births and deaths should be reported within the period prescribed by law / state rules.
- Late registration: If you miss this time, extra documents, affidavits or approval may be needed and late fee may apply.
- Corrections: For spelling mistakes or small corrections in existing certificates, written application with proof (ID, school record etc.) is required.
- Special cases: Adoption, foreign birth, court order and other complex matters are handled as per separate instructions from competent authority.

When You Should Visit the Office
- You do not have access to online facility or face difficulty in using it.
- Registration is very late and needs special permission.
- There is a mistake in name, date, address or other details in existing certificate.
- Case is complicated (adoption, foreign birth, court order, police case etc.).
